Coaches: Where to Look for Information to Complete Your Team's Packet for Districts

When turning in team packets for District Qualifying Tournaments ("DQTs") on Wednesday, April 9, several teams submitted incomplete entry packets.

Incomplete Rosters and Missing Code of Conduct Forms
Entry packets with incomplete or improperly filled in rosters and/or missing Coaches’ Code of Conduct forms.  The two most common missing items are listed below, along with a link to where to get blank copies of the form on this website:

Missing Documentation for Non-Potomac Valley Players
Also, many entry packets with players residing outside of the Potomac Valley PV AAU District boundries (click here for definition of the Potomac Valley District boundries) did not have specific documentation indicating that athlete’s district of residency, especially if the Potomac Valley District is to be the District of record with a non Potomac Valley address.

In those cases, school or guardianship documentation is required to establish residency. In cases where the player is outside of the PV AAU, then a simple statement should be noted that the athlete is declaring their home district as their district for eligibility determination.  Click here for the AAU’s rule on residency.
