Deadline for DQT Registration
All required material must be turned in to the Potomac Valley Girls’ Basketball Committee no later than 6:00 pm April 9, 2008 at the PVAAU District GBB Committee/DQT Draw meeting at Falls Church High School. The draw will begin at 7: 30pm. Room Location at FCHS: TBD.

Mailed packets must be received by April 7, 2008 or else the packets must be delivered to the April 9 meeting at Falls Church High School.
Required Documentation for DQTs
Player Requirements
Players are required to provide to their team the following information and documentation so that the team can complete its team entry packet with the Potomac Valley AAU Girls’ Basketball Committee.
· Signed AAU card (if not held by team manager or coach); the card must be signed by both the player and a parent/guardian
· Copy of Birth certificate with state seal
· Address of declared residency per AAU By Laws, Section 4.3; click here for the definition
Team Requirements
Teams are required assemble the following required information to provide to the Potomac Valley AAU Girls’ Basketball Committee before 6:00 p.m. on April 9, 2008:
· PV AAU GBB 2008 DQT Team roster form; click here to download the roster form.
· Copies of birth certificate with state seal from every player
· Signed AAU card for each player and address of declared residency if different form addresss of record on AAU Membership card and the AAU By Law section for same along with proper substantiation such as a custody agreement, school attendance letter, etc. per AAU By Laws, Section 4.3
. Payment for tournament fee; the $475 check should be made out to the club hosting the DQT.
Coaches’ Requirements
Each coach must sign and turn in a signed Coaches’ Code of Conduct form. Click here to download the form. If the form is not included with the team packet, it will need to be filed when registering your team on-site at the team’s DQT.

Note. The Tourney Packet section of the letter omits the residency documentation noted above in the 3rd bullet point under Team Requirements. Be sure your team packet includes the required residency documentation.
